We’re working on behalf of a medical clinic that was established to provide a turn-key operation for Physicians and Nurse Practitioner (NP) who are interested in joining a m! edical clinic in either a fee for service model or as part of a Family Heath Organization. Their medical clinic offers a brand new, state-of-the-art, 3,000 square foot medical hub, assembled with physicians, specialists, nurses and technicians. The organization provides exceptional and contiguous healthcare, featuring clinics currently underserviced in Kitchener-Waterloo.
We seek two (2) full-time Nurse Practitioners to join a brand new facility, fully-equipped with latest diagnostic equipment, running an easy-to-use EMR system(Practice Solutions, TELUS Health). In this role you will function in an expanded role and will demonstrate a high level of autonomy as well as expert skill to formulate clinical decisions, manage acute/chronic illness, and promote wellness. You will conduct comprehensive geriatric assessments; diagnose; provide treatment (including diagnostic, pharmacological and non-pharmacological therapies and counseling); develop, implement and document ! a plan of care in collaboration with the geriatricians and int! erprofessional team; provide education; refer to/liaise with other community services; and, provide follow-up as appropriate. In addition, you may be involved in other clinical, research, evaluation, leadership and collaborative activities (e.g. developing new standards of care, policies, etc.).
Candidates must hold registration in the Registered Nurse Extended Class with the College of Nurses. Must have membership in the Registered Nurses Association of Ontario. Preference given to candidates who hold Canadian Nurses Association certification in gerontological nursing. Must have demonstrated experience with completing comprehensive geriatric assessments. Must have demonstrated leadership skills, strong interpersonal communication skills, and the ability to establish positive relationships with all staff, clients, families, and community agencies. Current Basic Cardiac Life Support (BCLS) certification required. Must have valid Ontario Driver's License, relevant in! surance, and use of own vehicle as well as a willingness to travel throughout the Mississauga Halton Local Health Integration Network.
